PAYMENT OPTIONS

Sliding Scale
We offer a sliding-fee scale based on income and family size.

Insurance
We welcome public and private insurance plans, including Oregon Health Plan, Medicare and others.

Cover Oregon Enrollment Assistance
We have Access Specialists trained to help our patients with the Cover Oregon application and enrollment process. They are knowledgeable advocates who assist our patients in accessing affordable healthcare.

Income Verification
Household income verification is required to receive any discount. You will not be able to receive a discount until you provide verification of income. Income verification needs to be updated every six months.

Include income from all members of the household that are supported by that income. DO NOT include income from roommates, housemates, or parents if you are an adult over 19 living in your parents home.

Acceptable forms of income verification:

  • Pay stubs for the current month and the last two months
  • Check stubs for child support/alimony checks, unemployment, DHS/AFS, SSI, financial aid, etc.
  • Current tax return

For patients with no job or other income source:

  • A letter from an agency, friend, relative or past employer who knows the situation and is not living with the applicant
  • The letter must include the writer’s name and address (phone number if available) as well as a current date

COVER OREGON ENROLLMENT ASSISTANCE

Community Health Centers of Lane County (CHCLC) offers Cover Oregon enrollment at various clinic sites. Trained Access Specialists are available to meet with individuals and families to help them understand their health coverage options, and assist with the application and enrollment process. To schedule an appointment with an Access Specialist, please call: (541) 682-3585.

 

Cover Oregon includes health insurance from both private insurers and public medical programs, like Oregon Health Plan and Healthy Kids. Visit CoverOregon.com to learn more.

Cover Oregon applicants should be prepared with the following information at the time of their appointment:
~ Social Security number and citizenship or immigration information(if available) for everyone who is applying for coverag
~ Amounts of taxable income, and deductions or adjustments for 2014.
~ Insurance plan information (for each person on the application) about any current health coverage or health insurance available through an employer.

If the applicant does not have access to this information, they can still apply.
