2008 LCPH Flu Clinic Schedule

Friday, October 3rd,  8:30 to 11:30am and 1 to 4pm

Friday, October 10th, 8:30 to 11:30am and 1 to 4pm

Friday, October 17th, 8:30 to 11:30am and 1 to 4pm

Friday, October 24th, 8:30 to 11:30am and 1 to 4pm

Friday, October 31st, 8:30 to 11:30am and 1 to 4pm

 

            Cost:  $25 for adults

 

Insurance: 

LCPH can accept and bill the Oregon Health Plan and most Medicare cards. 

LCPH can not bill Medicare HMOs. 

You must bring your Oregon Health Plan card and Medicare card to the flu clinic for billing.

Children six months of age and older can receive flu vaccination at LCPH by appointment by calling 682-4041

 

 

Due to budget cuts, there will be no off-site community flu clinics this season

Parking is on the street at meters and in parking lots that have charges.  Be prepared to pay for your parking to avoid a ticket.
