Lane County Department of Health & Human Services is seeking proposals from providers to provide housing navigation and case management services. This project seeks to establish at least 4 FTE outreach workers and 1 FTE supervisor who will provide strategic, targeted, outreach services to approximately 100-150 single adults residing in City of Eugene Rest-Stops and microsites to help move participants toward housing and overall increased stability. The anticipated start date for this project will be January 1, 2020 2021. This work will be done in close coordination with Lane County Human Services Division (HSD) and City of Eugene. The team will engage with individuals staying in the rest stops, work to address immediate health and safety needs, decrease the spread of Covid-19, have problem solving conversations, assist in accessing benefits, and assist in coordinating housing support services with the primary focus on obtaining stable, permanent housing. The team will also collaborate with community, City and County partners to best meet the needs of this population. Multiple providers may be considered for this opportunity.

This project is intended to ensure progress toward meeting Lane County's Continuum of Care system performance measures as they relate to outreach services, in particular, reducing the number of people experiencing street homelessness and reducing length of time homeless.
