2019 CHSA - Social Determinants of Health

The social determinants of health include the conditions in which people are born, grow, live, work and age; and significantly influence the health status of individuals and communities. Socioeconomic factors such as income, poverty, food security, and education are strongly correlated to health outcomes.

Understanding how a community is faring on the social determinants of health is necessary to determine the best ways to go upstream to change the health status of the community as a whole. Also, many social determinants vary based on race/ethnicity and/or geography, creating significant inequities within Lane County.
