I have more than one individual in my family home but would likely only need one generator for the household. How should I reflect that on the survey?

Please fill out the survey for each supported individual in the home that would be supported by emergency supplies. We will review household needs as survey responses are gathered.

How will I know my application was received?

You should receive a confirmation e-mail after you submit your response.

You will also receive a confirmation e-mail from [email protected] with next steps and what to expect.

Oops, I made a mistake on my survey. How do I fix this?

E-mail [email protected] with your needed correction and we will update your survey.

Do you have information on what types of generators will be purchased with this grant?

We are currently seeking vendors for a variety of generator types. We hope to be able to provide options in generators as we understand one size may not work for all situations or needs.

Will we receive funding directly to purchase generators and/or emergency supplies?

Due to the nature of our funding source, all expenditures need to be tracked and recorded. Lane County is working to identify a vendor for ordering. Once the vendor is identified, notification will occur to confirm requested information and ordering will occur directly between Lane County and vendor for delivery to address provided in your survey.

What is the Prime Number the application asks for?

Your Prime Number is your OHP ID listed on your medical ID card.

What kinds of things can this grant cover?

We are allowed to make purchases of generators, power banks, and emergency supplies with this grant.

Is this grant open to all families or only ones with a need for medical equipment?

This grant is open to all individuals receiving services from Lane County Developmental Disability Services.

I applied for the grant. What happens next?

We are currently in the process of collecting survey responses and setting up our vendors for supplies. Once we complete this process, ordering can begin. You will receive an e-mail confirmation from [email protected] with what to expect as survey responses are processed.
