Personal Support Worker (NEW)

In order to start the process to become a new PSW you must start by scheduling a background check through our office. Even if you think you've done a background check recently or work through another agency. It is always best to call our office and inquire.

To schedule a background check call Drew Pryor at 541-682-2638.

At this appointment you will fill out a background check request form 0301 and a provider enrollment application and agreement form 0734. 

Personal Support Worker (RENEWAL)

If you are within 120 days from your credentials expiring it may be time for you to schedule your next background check.  As a courtesy, our office provides a 120 day notice to PSW's but it is the sole responsibility of the PSW to be aware of expiration dates and to act on it accordingly.

PSW background checks can take up to 12 weeks to clear and starting the process early will help you from experiencing a delay in your pay. If your PEAA and/or CHC lapses before you renew, then the period in between your expiration date and when the renewal is approved will NOT be billable.

At this appointment you will update and renew both your background check using form 0301 and provider enrollment application and agreement form 0734. 

FAQ's

Q: Is there a cost to me for this background check?

A: No, however, If you provide us with information that is a qualifier for fingerprints; i.e. out of state residence in the last 5 years or any criminal history, fingerprints may be required. If fingerprints are required, you will be instructed to schedule an appointment for Livescan with a Third Party agency. You must be prepared to pay $12.50 by credit/debit card at the time that you schedule this appointment. Appointments for fingerprints should be made within two days of being notified that you need fingerprints.

Q: What will I need to bring with me to this appointment?

A: For ALL Background Checks SI’s must bring a valid state issued photo ID, for most it will be a driver’s license but other acceptable forms include state issued ID, passport, social security card, or others (like birth certificate or military ID).

Q: How long do my credentials last once I am approved?

A: If the state approves your background check then you may work for two years as a paid provider. If you begin fulfilling other job duties than which your current background check is approved for, you may be required to fill out paperwork for a new background check.

Adult Foster Care

To complete a Adult Foster Care Background Check call Corie Fitzgerald at 541-682-6780.

Bring valid photo ID to your appointment. When you arrive for your appointment, check in with the front desk and let them know you have an appointment for a Criminal History Check.

If the state approves your background check then you may work for one year as a paid provider. If you begin fulfilling other job duties than which your current background check is approved for, you may be required to fill out paperwork for a new background check.

Child Foster Care

To complete a Child Foster Care Background Check call Corie Fitzgerald at 541-682-6780.

CFC Background Checks through our office are only processed for providers and each Subject Individual will require fingerprints after the appointment. For more information contact Corie.

Behavior Consultant

To complete a Behavior Consultant Background Check call Corie Fitzgerald at 541-682-6780.

Other Important Information

A less than spotless background check may not disqualify you to work as a paid provider for Lane County.

Presenting fraudulent information on your Criminal Background check may disqualify you from working as a paid provider for Lane County.

If the State declines to approve your Criminal History check then you may not work as a paid provider for Lane County.
