Your phone appointment requires the following forms that can be read and signed through our secure web portal.

Other needed information:
  • Eligibility Proofs: Income/Address & ID for you and your child(ren). 

  • Height, weight, and iron test results (HGB) for person appointment is for: You can use information from a doctor visit or other health care provider.  Screenshots of your online medical record or medical summary sheet are preferred.

For scheduled appointments, our calls will come between the scheduled time and up to 15 minutes afterwards.  For example, if you are scheduled for a 10:00am appointment, your WIC call will come between 10:00am and 10:15am.  We will try to reach you twice during that time period.

Once staff reach you, your appointment may last from 25-40 minutes per person.

If you have questions, please call us at 541-682-4202.  Please leave a message if your call is sent to our voicemail.  WIC staff will call you back.  Messages left outside of our regular hours will be  called back the next business day.
